Definition
  • Ejaculation ≤1 min of penetration for ≥6 months
  • Causes sexual frustration, distress, relationship issues
  • No routine labs unless indicated

  • Degree of stress/bother
  • Onset & duration
  • Medical & physical exam
  • Psychosexual issues

Treatment
  • Counseling & education
  • Squeeze technique + Stop-start
  • Second attempt coitus may improve control
  • If due to ED → treat ED first
Approved On-demand Therapy
  • Dapoxetine 30–60 mg 1–2h before sex (once/24h)
  • Lidocaine/prilocaine spray (may numb partner)
Daily / Off-label Therapy
  • Fluoxetine 20 mg OD
  • Escitalopram 10 mg OD
  • Topical anesthetics
Course: 4–6 weeks, max 12 weeks
